Technical Assistance:
Discussion with on site staff in regards to certificates and what needs to be on site for both staff; and available for review by licensing.

A review of the updated allergy list was conducted.

Comments:
This monitoring inspection was initiated by licensing staff and concluded on 09/14/2023, between the hours of 2:30 pm to 4 pm. There were 28 school-age children present, with 2 staff supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication; special care and emergencies; and nutrition. A total of 5 children records were reviewed. Staff records are reviewed on an annual basis through the Loudoun YMCA admin. office in Leesburg by a representative of the Office of Child Care Health & Safety (Fairfax).

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the center.

Violations:
Standard #: 8VAC20-780-510-B
Description: Based on review and interview, 2 medications on site did not have a written medication authorization on file signed by both the parent and child's doctor.

Evidence -
1. On the date of inspection (09/14/2023), two medications prescribed to Child #1, was without their medication authorizations. Both medications were to be administered on an as needed basis for a listed allergy.
2. The Director stated (during an interview on the date of inspection) that it was believed that this authorization may have been received, but was currently not within the child's file.

Plan of Correction: This written medication authorization will be found and returned t0 the child's file, or a new authorization will be requested with signatures from both the parent and child's doctor.

Standard #: 8VAC20-780-550-D
Description: Based on review, practice evacuation drill were not implemented on a monthly basis.

Evidence -
1. On the date of inspection (09/14/2024), there was not a documented record of drills having been conducted for the months of March, April, May, and June of 2023.
2. The current drill log for this school year does reflect a drill being conducted in August 2023.

Plan of Correction: The facility, going forward, will conduct fire drills on a monthly basis, shelter-in-place drills twice a year, and a lockdown drill on an annual basis.
