Comments:
An unannounced Monitoring Inspection was conducted on August 14, 2019 from approximately 10:40 am to approximately 2:35 pm. Upon arrival there were 115 children present with 19 staff. There were additional staff on site for administrative and program support. The infants were observed eating, freely playing and during diaper changes. Children were also during story time, eating lunch and playing in centers to include blocks, puzzles, computers and during their nap time. The sample size of records reviewed included 10 children's records and 8 staff records. Eight medications were reviewed as a part of the inspection. Areas of non-compliance are noted on the violation notice. The results of the inspection were reviewed and verified with the Assistant Center Director and center owner during the exit interview. Based on a review of ten staff records and staff interviews, the licensee did not ensure that all staff had obtained a copy of the results of a search of the central registry and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him.
Evidence: The record for staff #5 did not include the search of the central registry for the state of Illinois where they had lived within the last five years.
Plan of Correction:
The Assistant Center Director will have the staff fill out the form today.
