Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 09/10/2021 and concluded on 09/16/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 12 children present, ranging in ages from 15 months to 4 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1) The records for Child #1 (start date 08/16/2021) and Child #2 (start date 09/18/2020) did not contain the results of a physical examination.
Plan of Correction:
Will contact the parents to request the physical examination. Child #1 has an appointment on 10/15 for the child's annual review.

Based on record review, children's records did not contain all required information. Evidence: 1) The record for Child #1 did not contain the name, address and telephone number of a second person to contact in case of an emergency if the parent cannot be reached.
