Technical Assistance:

Comments:
An unannounced monitoring inspection was conducted at 11:30am. Observed 12 children totaling 32 points with 3 caregivers. Ratios and supervision were in compliance. Children were eating lunch and taking naps during the inspection. Lunch included: chicken nuggets, pasta, green beans, bananas, grapes and milk. The caregivers were actively engaged with the children and promptly tending to the needs of the children. The daycare areas of the home were found to be clean and sufficiently supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Based on records reviewed, 1 of 3 caregiver records did not include a required background check every 5 years. Evidence: The record for Caregiver #2 did not include a completed sworn disclosure statement in the last 5 years. The statement on-file was dated 12/12/17.

Description:
Based on records reviewed, the emergency plan was not reviewed by the licensee annually. Evidence: The plan was last reviewed 3/28/22.

Description:
Based on observations made, children under the age of 4 years old were served food in a manner that did not prevent choking. Evidence: Whole grapes were served to the children between ages 2-3 years old.
Plan of Correction:
The provider said grapes were cut for the children under 2 years old and they will cut the grapes for all children in the future.

Description:
Based on records reviewed, 1 of 3 household members did not have a completed TB screening every 2 years. Evidence: The TB test for Household member #2 was dated 7/28/21.
Plan of Correction:
The provider said a TB test will be scheduled promptly.
