Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:30 am through 10:45am with the provider on site. There were 3 children (6 points) with the provider. A sample of 5 children's files and 1 staff/household member's files were reviewed. The children were engaged in the following: toy play, snack, bathroom breaks, hand washing, and other organized activities. Interactions between the children and provider were positive. The physical space, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Based on review, four of five children's records did not have documentation of the yearly review of the child's emergency contact information as required. Evidence: 1. Child #2, Child #3, Child #4 and Child #5 had been enrolled for more than 1 year and did not have documentation of the yearly review of the emergency contact information.
Plan of Correction:
I will ask the parents to reveiw and document today.
