Comments:
An unannounced monitoring visit was conducted today. Three children were present (11 points) with two caregivers. Reviewed children, caregivers and household members records, emergency preparation, physical space, training hours and care of children. Children bring lunch from home. Please ensure crib sheets are tight when cribs are occupied. Areas of non-compliance are identified in the violation notice. Based on observation, over the counter products were not labeled with the child's name. Evidence: Two diaper creams were missing the child's name to identify the owner.

Description:
Based on record review, 1 of 3 children's records was missing an immunization update. Evidence: Child #1 is under 2 years of age and does not have a current immunization record within the last 6 months (7/25/2018)
Plan of Correction:
A new immunization record will be requested from the parent.
