Comments:
The licensing inspector conducted an unannounced monitoring inspection on 09/26/2019 from 9:00am to 12:10pm. The children were observed learning through play in centers, reciting the days of the week, identifying letters, playing on the playground and eating breakfast. Breakfast consisted of french toast sticks, fruit juice and milk. A formal observation was completed with two preschool classrooms. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms were inspected today as well as the playground.
Five children's files were reviewed. The staff files are located at another site and will be reviewed on a later date. Medication is being administered and was reviewed.
The center's first aid kit and emergency supplies were inspected. Last emergency drill: 09/20/2019 Last shelter-in-place drill: 09/10/2019 Last fire inspection: 9/6/2018 Last health inspection: 05/08/2019
Today, the following child to staff ratios were observed: Rm. #3 (Fours) - 15:2 Rm. #4 (Fours) - 13:2 Rm. #5 (Threes) - 12:3 Rm. #6 (Fours) - 15:2
The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.

Based on record review, the center did not ensure to have documentation of an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: The center had documentation of a fire inspection report dated 9/6/18. The center's representative stated that they have contacted the fire official and an inspection should be completed today.
Plan of Correction:
We have been in contact with the Fire Marshal and we have a fire inspection scheduled today at 1:00pm.
