Comments:
An unannounced monitoring inspection was made on 6/25/19 in response to a self-reported incident on 6/11/19. The incident involved a forbidden action toward a child from a staff person. A joint investigation with local social services was conducted on 6/25/19 and 7/9/19. Staff interviews were conducted and video recordings were observed. Based on the investigation, there was a violation cited related to this incident. The investigation was completed and the inspection report was sent to the facility representative on 7/25/19.
Based on review of a video recording and interviews, the center failed to ensure that physical punishment, striking a child, roughly handling a child was used as forbidden by the Standards.
Evidence:
1. A video was reviewed of an incident on June 10, 2019. The video showed activities of children and staff in the toddler room on the morning between 10:00 am and 11:00 am. A staff person was observed to pick a two year old child up by one arm forcefully and drag/carry the child to another part of the classroom for time out, according to staff interviews.
2. Staff interviews with staff #1, #2, and #3 revealed that the child was being corrected for unacceptable behavior and was physically taken to time out.
3. Staff #1 stated that after review of the video, the actions "shocked" the staff person of how forceful the action of staff #1 looked.
Plan of Correction:
Staff #1 employment was terminated. Immediate training was put into place by the administration to cover the topic of handling children. The administration has requested additional training from the Licensing Inspector for all staff in August.
