Comments:
A complaint was received by the Division of Licensing Programs on 11/14/19 regarding concerns of the following: medication administration of an infant, emergency procedures, ratio in the early morning hours for the older infant/toddler age children, medication authorization for an emergency medication not being available at the center.

An unannounced complaint inspection was initiated on 12/6/19 at 7:05 am to observe the early morning ratio in the older infant/toddler room. All of the other parts of the complaint were reviewed and investigated through documentation review, review of policies and procedures, and interviews. There were no violations in any of the areas other than medication administration. The complaint was found to be valid for the medication administration. Additional review of records was completed on 12/30/19. The complaint was completed and sent to the center administrator via email on 1/6/20.

Description: Based on record review and interviews , the center failed to ensure that a prescription medication was administered to a child identified on the prescription label in accordance with the prescriber's instructions pertaining to dosage, frequency, and manner of administration.

Evidence:
1. A written authorization signed on 8/14/19 by the parent and by the physician of child #1 was observed for a prescription eye drop to be given four times a day, specifically to be administered to child #1 at the center at 12:30 each day. The attendance log for child #1 was compared to the administration log for the medication. There were 56 days that the eye drop was not administered to child #1 in which the child was present at the center during the period of 8/14/19-12/4/19. Child #1 was 13 months old when the medication was missed in August (14 months in September and 15 months in October).

2. Staff #1 verified that the medication was only administered when the MAT (Medication Administration Training) certified staff person present at the center was notified to administer it. Staff #1 who is MAT certified staff person stated that she was not notified on the dates that the medication was not given. Staff #1 was the designated staff person to administer the medication for this child.

Plan of Correction: The eye drop will be administered daily as required.
