Comments:
An unannounced monitoring inspection was conducted today between the hours of 9:39am through 11:15am with the provider on site. There were 12 children (29 points) with the provider and two assistants. A sample of 5 children's files and 4 staff/household member's files were reviewed. The children were engaged in the following: toy play, circle time, singing, coloring, snack, bathroom breaks, hand washing, and other organized activities. . The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and caregivers were positive. The physical space, emergency supplies, evacuation drills, attendance records, injury reports and the Emergency Preparedness Plan were reviewed. Based on record review, the family day home did not obtain documentation of additional immunizations for a child once every six months for children under the age of two years.
Evidence:
1. Child #4 had an immunization record dated 4/10/2017 and Child #5 had an immunization record dated 8/30/2018 and needed an updated immunization record on file.
Plan of Correction:
I contacted the parent and requested an updated immunization record during the inspection.
