Comments:
An unannounced monitoring inspection was conducted on November 19, 2019. Upon arrival, there were 167 children present . Additional staff were on site for administrative and support purposes. The children were observed eating lunch, listening to story and napping. Licensing inspector reviewed supervision, activities, equipment, diapering and hand washing procedures, injury report documentation, medication, children and staff records, emergency evacuation and shelter-in-place drill documentation, emergency supplies and required postings. Seventeen children's records and seven staff records were reviewed. three medications and five injury reports were reviewed. The exit interview was conducted with the program director.

Standard #: 22VAC40-185-70-A
Description: Based on record review, the licensee did not ensure that each staff record was maintained with all required information.

Evidence: The record for Staff #4 and Staff #6 did not have orientation documentation.

Standard #: 22VAC40-185-330-B
Description: Based on observation and inspection of the facility, the licensee did not ensure that where playground equipment is provided, resilient surfacing shall comply with minimum safety standards and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles. Fall zones are defined as the area underneath an surrounding equipment that requires a resilient surface. A fall zone shall encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.

Evidence: The resilient surfacing (rubber mulch) on Preschool playground #1 and Preschool playground #2 the resilient surfacing was measured in multiple areas of the playground were approximately 1inch to 5inch of mulch . The center director confirmed there was not enough resilient surfacing under and around the play ground composites.
