This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and interviews.
A monitoring inspection was initiated on 3/31/2022 and concluded on 4/04/2022. A self-reported incident was received by the department regarding allegations in the areas of supervision and forbidden actions. The supervisor was contacted by telephone to conduct the investigation. Staff and one child were interviewed to complete the investigation.
