Discussed Safe Sleep Practices and documentation of program leader qualifications.
Comments:
An unannounced, on-site renewal inspection was initiated on July 15, 2024 and completed on July 15, 2024, as a part of licensure period. The on-site inspection began at 11:15am and ended at 1:34pm. The inspector reviewed compliance in the areas listed above. There were 59 children present and 11 staff. The inspector reviewed 8 children?s records and 5 staff records on-site July 15, 2024. This inspection included document review, tour of the facility, interviews and observations
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 26, 2024. A POC submitted after this date will not appear on the public website.
Based on record review, the licensee did not ensure before child attend the cneter recieve documentation of required immunization.
Evidence : The record for child #8 (start date 7/1/2024) did not have required documentation of immunization prior to attending the center. Confirmed by Director.

Evidence:
1- The record for child #1 was missing information for previous child day care and schools attended by the child; name of any additional programs or schools that the child is concurrently attending and the grade or class level.
2- The record for child #6 was missing on both emergency contacts city, state and zipcode.
3- The record for child #7 was missing addresses for both emergency contacts.
4- The record for child #8 was missing physican phone number, allergies and intolerance to medication or any other substances, and actions to take in an emergency situation; previous child day care and schools attended by the child; name of any additional programs or schools that the child is concurrently attending and the grade or class level and documentation of viewing proof of the child's identity and age.
