Comments:
An unannounced complaint inspection was conducted 07/26/2023. The inspector interviewed 3 staff members, reviewed one child?s record and 1 injury report, during the investigation. The inspector arrived to the center at 9:50 am and departed at 1:20 pm. The evidence gathered during the inspection determined the complaint to be valid.
Information gathered during the course of the investigation determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on staff interviews and document review, the center failed to maintain a written record of children?s serious and minor injuries in which entries are made the day of the occurrence and that the record shall include all of the elements required by the standard.
Evidence: 1. Staff 1 and Staff 2 verified that there had been three incidents, one on 05/09/2023, one on 05/10/2023 and one on 05/11/2023, that required an injury report to be completed. Staff 1 stated they had completed the injury reports. There were no injury reports from 05/09/2023 or 05/11/2023 available for review during the 07/26/2023 inspection. 2. The injury report from 05/10/2023 did not include the following required elements: any future action to prevent recurrence of the injury.
Plan of Correction:
All injury reports must now be verified by the director or the acting director.
