Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 3/4/2021 and concluded on 3/5/2021. The director was contacted by telephone to initiate the inspection. There were 45 children present and 9 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 5 children?s records and 4 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Violations:
Standard #: 22VAC40-185-510-A
Description: Based on review of medications, it was determined that 3 of 4 medications were on site without a signed medication authorization form.

Evidence: Child #5 has two emergency medications on site without a medication authorization form signed by the parent. Child #6 has one emergency medication on site without a medication authorization form signed by the parent and physician.

Plan of Correction: The forms were given to the parent for a doctor's signature and the parents have signed the forms.

Standard #: 63.2(17)-1720.1-B-4
Description: Based on record review, the center did not obtain the results of an out of state sex offender name search for each staff member who has resided outside the state of Virginia within the last 5 years.

Evidence: Staff #2 (Date of Hire: 3/14/11) has resided in Maryland within the last 5 years and did not have a sex offender name search completed by 12/31/2020 as required for existing staff members. Staff #3 (Date of Hire: 2/15/21) has resided in New York within the last 5 years and did not have a sex offender name search prior to first day of employment as required for new staff members.

Plan of Correction: Each of these were run and a screen shot was sent to Ms. Bryson on 3/8/2021.
