Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated and concluded on October 12, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 31 children present, ranging in ages from 2 years old to 4 years old, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-185-510-A
Description: Based on record review and interview, the center did not obtain written authorization from the child's physician and a parent.
Evidence:
1. Child #1 has emergency medication on site. There was no documentation of written authorization from the parent.
2. Child #2 has emergency medication on site. There was no documentation of written authorization from the physician and parent in the record.
3. Staff #1 stated that parent and physician authorization was not obtained for emergency medication on site.

Plan of Correction: The center will give the parents a medication authorization form to be completed by the parent and physician.
