Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on October 12, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 31 children present, ranging in ages from 2 years old to 4 years old, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and four staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview, the center did not obtain written authorization from the child's physician and a parent. Evidence: 1. Child #1 has emergency medication on site. There was no documentation of written authorization from the parent. 2. Child #2 has emergency medication on site. There was no documentation of written authorization from the physician and parent in the record. 3. Staff #1 stated that parent and physician authorization was not obtained for emergency medication on site.
Plan of Correction:
The center will give the parents a medication authorization form to be completed by the parent and physician.
