Comments:
A Subsidy Health and Safety inspection was conducted on 10/21/2019 to review supplemental health and safety requirements. There were 28 children, ages 5 year-11 years, and 4 staff members present during the inspection. The inspector reviewed 5 children's records, the on-site staff record requirements for 5 staff members and 1 medication during the inspection. Children were observed in the following activities: participating in organized play in the gym, having an afternoon snack, during homework time and participating in arts and crafts. The inspector reviewed the emergency preparedness plans and the emergency supplies during the inspection. The inspector discussed children's allergies and table sanitizing products with the staff. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and can be found to the Violation Notice. Based on record review and discussion with the staff, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.
Evidence: Child #6 has a diagnosed food allergy. There was no written allergy care plan for Child #6 available during the inspection.
Plan of Correction:
The site director will get a plan from the parent.
