Technical Assistance:
Discussed with the provider to update medication policy to reflect that they will not administer medication since the MAT certification has not been renewed. (There were no medications on site on the date of inspection)
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 10/25/2021 and concluded on 10/28/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 8 children present, ranging in ages from 3 months to 5 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 3 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, children's records did not contain all required information. Evidence: 1) The record for Child #1 did not contain a telephone number of one custodial parent's place of employment.

Description:
Based on documentation review, the provider did not ensure that wood burning fireplaces and associated chimneys were inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. Evidence: 1) The most recent fireplace inspection was dated 01/18/2020.
