Comments:
A monitoring inspection was conducted on 10/25/2023 between the hours of approximately 11:25 am and 12:10 pm. There were 12 children present, ranging in ages from 4 months to 3 years, (28 points) with 3 caregivers supervising. Children were observed while playing outside, handwashing, and listening to a storybook.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-800-120-B
Description: Based on record review, one caregiver record did not contain all required information.
evidence:
1) The record for Staff #4 did not contain the following: Name address and telephone number of a person to be notified in an emergency; Address; Date of employment.

Plan of Correction: Staff #4 completed the emergency form on 10/25/2023.

Standard #: 8VAC20-800-200-A
Description: Based on record review, one caregiver did not complete required orientation training by the end of their first week of assuming job responsibilities.
Evidence:
1) The record for Staff #4 (start date 09/05/2023) did not contain documentation that the assistant completed orientation training as required.

Plan of Correction: Staff #4 completed the orientation training on 10/25/2023.
