Technical Assistance:
The Code of Virginia states: Each building that was built before 2015 and that houses a child day program that is licensed pursuant to this chapter or any program described in subdivision A 4, B 1, or B 5 of ? 22.1-289.030 that serves preschool-age children shall be equipped with at least one carbon monoxide detector.

The licensee was not incompliance with the above requirement during today?s inspection.

Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews..

A renewal inspection was initiated and concluded on 11/23/2021. The provider was contacted by telephone and a virtual inspection was conducted. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records, 2 staff records and 1 household member record were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review and interview, the provider has not obtained documentation of a physical exam for each child within the first 30 days of enrollment.
Evidence: Child #2 (date of enrollment: 10/13/2021) file does not contain documentation of a physical exam. The provider confirmed that this family has not yet provided a physical exam.

Plan of Correction: I will text the parents today and tell them they need to bring it in next week.

Standard #: 22VAC40-111-60-B
Description: Based on record review and interview, children's records did not contain all required information.
Evidence: Child #4's file does not contain addresses for two out of two emergency contacts.

Plan of Correction: I will have the parents fill the information in this week.
