Comments:
An unannounced renewal inspection was conducted today between 10:20am and 11:40am. There were 6 children present with the Provider and 1 assistant for a total of 16 points within the supervision guidelines. The children were observed playing freely with trucks and dancing. A complete inspection of the physical plant, children and staff records, fire drill log, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on record review the provider did not review the emergency plan annually. Evidence: The most recent documented review of the providers emergency preparedness plan was dated 9/3/2020.

Description:
Based on record review and interview, the provider has agreed to administer emergency medication and did not renew medication administration training every three years. Evidence: The providers medication administration training expired 10/13/2022. The provider confirmed that their training had expired.
Plan of Correction:
I took the online part of the training and I'm waiting for the second in-person part of the training. I'll contact the trainer today. She said she'll have a training in December.

Description:
Based on observation and interview, areas and furnishings of the family day home, inside were not maintained in a clean, safe, and operable condition. Evidence: 1. The wall outside of the childrens bathroom is dirty. 2. The door to the childrens bathroom is dirty. 3. There are areas of black dots around the vanity in the childrens bathroom. The provider confirmed that the black spots are mold. 4. The wall under the window is dirty with drip stains.

I'm having someone come this weekend to clean and fix it all.

Based on observation, potentially poisonous substances, such as cleaning agents were not stored inaccessible to children. Evidence: Various cleaning materials and sanitizers were stored under the sink in the childrens bathroom. The cabinet was accessible to children and unlocked.

Description:
Based on observation child-resistant protective covers larger than 1-1/4 inches in diameter were not installed on all unused electrical outlets and surge protectors accessible to children under five years of age. Evidence: 1. There were two uncovered outlets next to the childrens table and chairs. 2. There was one uncovered outlet next to the childrens feeding chairs.
Plan of Correction:
It's really hard to push the covers in. I'm going to have to replace the outlet cover.
