Technical Assistance:
Discussed the need for staff to be retrained on bottle feeding procedures.
Comments:
The Licensing Inspector conducted an investigation in response to a self report incident regarding a staff member giving the wrong bottle to an infant on 11/13/2019. Interviews were conducted with staff and staff and child record reviewed. The information gathered during this investigation supports violations being cited that were discussed with the center director. Based on multiple interviews and documentation, when staff are supervising children , they did not ensure their care, protection and guidance. Evidence: 1. Staff #1 and Staff #2 admitted that Child #1 was given Child #2's bottle of breast milk on November 13, 2019 by Staff #1. Staff #1 stated the parents of Child #1 and Child #2 were notified by phone and in writing on November 13, 2019 of the incident that the wrong bottle was given to Child #1. 2. Child #1 and Child #2's incident report form dated 11/13/2019 were reviewed and confirmed Child #1 was given the wrong bottle.
Plan of Correction:
Staff #1 was retrained on the steps for infant feeding and the confirmation of identity of the child. The staff will all be retrained on the infant feeding policy.

Description:
Based on observation, prepared infant bottles were not dated. Evidence: 1. Four infant bottles labeled with Child #2's name on it that were in the infant classroom refrigerator did not have the date on the bottles.
Plan of Correction:
The staff will be retrained in the infant classroom on the acceptance of food and bottles to ensure they have the proper label with child's name and date..
