Comments:
An unannounced inspection was conducted on May 9, 2024 from 11:15 am to 12:39 pm in response to a complaint regarding forbidden actions. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, there was a violation found during the inspection regarding staff qualifications and training. See the violation notice for more details.
Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it within 5 business days. Please specify how the deficient practice will be or has been corrected.
Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Based on review of one staff record (S3), and an interview with a center representative (CR1), the center failed to keep staff records as required.
Evidence: There was no documentation in the record that S3 completed 16 hours of training appropriate to the age of children in care annually. There was documentation of 9 hours of training between 1/20/23 and 2/26/24 for S3. In an exit interview on 5/09/24, CR1 acknowledged that 16 hours of annual training was not documented.
Plan of Correction:
Steps to correct the noncompliance with the standards 1) A teacher did take the classes required and more for the annual requirements. She sent them to our program director and they were not recorded. The program director is no longer with us. Measures to prevent the noncompliance from occurring again 2) The program director has been replaced with another program director that understands the need for accurate record keeping. Person responsible for implementing each step and/or monitoring any preventative measures. 3) The new program director is in place and back tracking all files for employees/teachers/aides as well as children in care to make sure all requirements are met and records are in place.
