An annual RECDC inspection was conducted on today's date between 10:45am-12:45pm. The church operates a camp and today the center had 147 children in care in six groupings with 26 supervising staff. All groupings were in ratio. The center has rooms with adequate space and supplies for the children to explore. The children were engaged in water activities, lunchtime, games, group time, and working in small groups with the counselors. The children bring their own food from home. Based on record review, the center did not ensure that each person in a supervisory position had been certified by a practicing physician or physician assistant to be free from any disability which would prevent him from caring for children under his supervision. Evidence: 1. Staff #1,2,3,4,5,6,7,8,9,10,11,12,13,14,15, and 16 did not have documentation of a Staff Health Report certified by a practicing physician or physician assistant in the records.

The Staff members will get a physician to complete the Staff Health Reports and email the Licensing Inspector.

Based on record review, the center did not obtain a fingerprint based criminal history record check prior to the first date of employment. Evidence: 1. Staff #12's record (Date of hire-06/16/2022) did not have results of a fingerprint based criminal history check.

The staff member will get the fingerprints completed immediately. Results will be emailed to the Licensing Inspector.

Based on record review, the center did not obtain a sworn disclosure statement prior to the first day of employment. Evidence: 1. Staff #1,7,8,10,14,15,and #16's records did not contain documentation of a Sworn Disclosure Statement.
Plan of Correction:
The staff members will complete a Sworn Disclosure Statement and email the Licensing Inspector. Moving forward, the staff members will complete the Sworn Disclosure Statements before the first date of employment.
