Comments:
An unannounced monitoring visit was conducted today. Five children were present (15 points) with two caregivers. Observed infant care, free play with toys at the daycare and rest time. Reviewed two children's records, trainings for the caregivers, emergency preparation and physical space. Lunch to be served today is lentils with carrots and rice, fruit and milk. Based on record review, 2 of 2 children's records did not contain all required information. Evidence: Child #1 was missing a second emergency contact with address and phone number Child #2 was missing an annual update as of 3/2019
