Comments:
An unannounced monitoring inspection was conducted on-site February 28, 2024 from 10:46am until 12:37pm. The director was available during the inspection. There were 42 children present with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation and interview, the center failed to ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.
Evidence: 1. In the two-year old class there were several cans of spray paint and a bottle of Goo Gone on a shelf in the bathroom. The label on the Goo Gone state "keep out of reach of children and danger."
2. In the three-year old classroom there were three cans of Chlorox wipes and Lysol spray in an unlocked cabinet in the bathroom. There were three cans of Barbosol shave cream on an art cart in the class accessible to children. The labels on each of these state "keep out of reach of children and caution."
3. In the four-year old classroom there were two cans of shaving cream, five bottles of contact lens solution, two bottles of hydrogen peroxide, and a can of Elmer's Craft Bond on a cart under the sink and accessible to children. The labels stated "keep out of reach of children and warning or danger."
4. Administration acknowledged the hazardous substances were not locked.

Plan of Correction: 1) Steps to correct the noncompliance with the standard(s); The Head of School (director) and Administrative Coordinator will email the staff to remind them of what the standard says and ask that each teacher go through their classrooms, thoroughly, to check for any other items that should be out of reach or locked up and act accordingly. The Head of School and Administrative Coordinator will also remind the staff that going forward they need to ensure that they read all labels and comply with this standard.
2) Measures to prevent the noncompliance from occurring again;
Add more locks to the available cabinets and ask that teachers use these lockable spaces to keep materials out of access to children.
3) The person(s) responsible for implementing each step and/or monitoring any preventative measures.
The Head of School (director) and Assistant Head of School.
4) Date to be corrected:
All materials are currently out of the classroom or locked up. On 3/14/2024 the center is closed and the staff will be having Parent Teacher Conferences, as well as a window of time to work in their classrooms. Locks will be added and materials will be reviewed with staff.
