This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated and concluded on October 8, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 9 children present, ranging in ages from 6 months old to 4 years old (20pts), with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed. If you have any questions, please contact me at Tanya.johnson@doe.virginia or call me at 703.371.5038.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have any questions, please contact me at tanya.johnson@doe.virginia.gov or call me at 571.423.6392.
Based on a review of records, the provider failed to obtain a current tuberculosis screening for each caregiver and household member every two years from the date of the first screening.
Evidence: Caregiver #1 did not have a current TB screening, the last screening documented was 2/23/2019. Household member #1 and #2's records did not have documentation of a current TB test or screening. The last documented test for HHM #1 was 2/25/2019 and the last test documented for HHM #2 was 2/21/2019.
Plan of Correction:
The caregiver and the household members will obtain TB test or screenings.
