Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated and concluded on September 7, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 43 children present, ranging in ages from 6 months old to 5 years old, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of four child records and four staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, children's records did not contain all required information. Evidence: 1. Child #1's (Start date-04/19/2021) record did not contain documentation of viewing proof of the child's identity and age.

Description:
Based on record review, two of four staff records did not contain documentation of all required information. Evidence: 1. Staff #1's (Date of hire-06/07/2021) record did not contain documentation of proof of education. 2. Staff #4's(Date of hire-03/12/2021) record did not contain documentation of proof of education.
Plan of Correction:
The center will contact staff members to obtain proof of education and document the record.
