Comments: An unannounced complaint investigation was conducted on 12/2/19 from 12:45pm-1:50pm. The Northern Licensing Office received the complaint on 11/25/19 regarding supervision of children. Interviews were conducted, records reviewed, and observations made throughout the physical plant.
