An unannounced focused monitoring inspection was conducted on 9/18/2023 from 11:10 am to 11:30 am. The licensing inspector reviewed five staff records. Some documents were emailed to the inspector after the onsite visit because of staffing issues on 9/18/2023. The final set of documents were received 9/20/2023.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on staff record review, the center failed to obtain subsequent background checks on staff, due every five years.
EVIDENCE: The most recent Criminal Record Fingerprint Eligibility letter for Staff 4 was dated 7/18/2018. Staff 6 showed a letter showing that fingerprints were scheduled to be done 9/19/2023.

EVIDENCE: Staff 1 began work on 6/21/2023 and their staff file does not have the results of a Virginia Central Registry check. Documentation in the staff file shows it was requested on 9/7/2023.
