An unannounced monitoring inspection was conducted Monday, October 28, 2019 from approximately 1:15 PM until 2:00 PM and Thursday, November 14, 2019 from approximately 9:30 AM until 10:40 AM to determine the center's compliance with religious exempt requirements. During the November 14, 2019 inspection a total of 20 children (ages three to six years of age) were present during the inspection and being supervised by two staff members. Children were observed creating maps of the United States, looking at books, writing, counting beads, playing with alphabet dice, working puzzles, using stencils, playing with play-doh, building with blocks, washing hands and taking bathroom breaks and eating snack. All areas of the center were inspected including the classrooms, hallways, bathrooms and outdoor play areas. Five children?s records and two staff records were reviewed. Policies and procedures were discussed and information disclosed to parents, guardians and public was reviewed. Violations were cited during the inspection. Based on a review of two staff records and interview, the center did not ensure there was at least one staff member present that had been trained to conduct daily simple health screenings to exclude children as necessary. Evidence: The two staff records did not contain documentation of training to conduct daily simple health screenings. The two staff members stated the children are screened daily for any symptoms that would require the children to be excluded but they have not completed an official training.
Plan of Correction:
I will take the online class offered that is acceptable for daily health screening.

Description:
Based on review of five staff records, the center did not ensure background checks for one staff member was updated at least every five years. Evidence: The record for Staff #2 (hire date: 08/22/2011) contained a central registry finding dated 08/24/2011 and a sworn statement dated 08/24/2011. Effective July 1, 2017, staff at all religiously exempt child day centers must repeat background checks at least every five years.

Description:
Based on review of two staff records and interviews, the center did not obtain satisfactory results of the fingerprint-based national criminal background check by 09/30/2018 for two staff hired prior to 1/22/2018. Evidence: 1. The record for Staff #1 (hire date 08/26/16) did not contain satisfactory results of the fingerprint-based national criminal background check. Staff #1 stated she has not scheduled an appointment as of 11/14/19 to obtain the national criminal background check. 2. The record for Staff #2 (hire date 08/22/11) contained satisfactory results of the finger-print based national criminal background check dated 11/04/2019. 3. Staff stated she thought ?employees with the Virginia State Criminal Record Check were grandfathered in and did not have to complete the fingerprint national criminal record check?. 4. Staff hired prior to 01/22/2018 was required to have the results of the national criminal background check by 09/30/2018.
Plan of Correction:
Staff #1 will schedule an appointment within the next two days to complete the required national background check.
