Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 1/22/2021 and concluded on 1/22/2021. The provider was contacted by telephone to initiate the inspection. There were 2 children present and 1 staff. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff/household records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review & discussion with the Provider, the Vendor failed to proof of identity and age for each enrolled child.
EVIDENCE: Licensing Inspector reviewed 2 children records. Records for both children did not include documentation of proof of age and identity. Both children were enrolled on 10/02/2019.
Plan of Correction:
Provider will obtain information from the parents.
