Comments:
The licensing inspector conducted an unannounced complaint inspection on February 18, 2020 from approximately 2:00 PM until 3:00 PM in response to a complaint that was received by the Department on January 14, 2020 regarding injury reports, daily activities and diapering. Staff interviews, observations and documentation were reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation, so the complaint is determined to be not valid. A violation was cited, not relating to this complaint, during the inspection. .

Based on review of one child's record, the center did not obtain documentation of immunizations as required by the State Board of Health before the child's first day of attendance at the center.
Evidence:
1. The record of child #1, enrolled 08/12/2019, did not have documentation of immunization on file. 2. The director acknowledged the record of child #1 did not contain documentation of immunizations.
Plan of Correction:
Per the director: I thought the immunization record was attached as indicated on the student health form, however they were not. I will check more closely to ensure all required information is obtained.
