Technical Assistance:
When fingerprint letter is received, please forward to your inspector. Caregiver cannot work until eligible fingerprint letter is received. A carbon monoxide detector is required to be in your home.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews (if applicable) and a virtual tour of the program (if applicable).
A renewal inspection was initiated on October 13, 2021 and concluded on October 14, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 10 children present, ranging in ages from 1 month to 5 years, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Child records and 2 caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on review of two caregiver's records, one does not contain documentation of a current report of tuberculosis screening form Evidence The record for staff #1 does not contain documentation of a current report of tuberculosis screening. Report in record expired in September 2021.
Plan of Correction:
I had the screening done, but haven't received the results yet. I will put them in my record when they are received.

Description:
Based on review of two caregiver's records, one does not contain documentation of a minimum of 16 hours of annual training in areas that are relevant to their job responsibilities. Evidence The record for staff #1 does not contain any documentation of annual trainings.
Plan of Correction:
Due to COVID I did not attend any in person trainings. I will try to do some on line.

Description:
Based on review of two caregiver's records, one does not contain documentation of an eligible fingerprint letter, before their first day of employment. Evidence The record for caregiver #2 does not contain documentation of an eligible fingerprint letter before their first day of employment.
Plan of Correction:
They had their fingerprint done somewhere else, she will have it completed from a Fieldprint office and I will put the results in her record. Results will be sent to inspector when received. Caregiver will not work until eligible letter is received.
