Comments:
An unannounced renewal inspection was conducted with the voluntarily registered provider on 10/24/23 from 9:45am to 10:40am. There were four children in care, ranging in age from eight-months to three-years-old, for a total of 10 points. Children were observed watching an educational show and eating graham crackers. An infant was observed napping and waking from nap. The Licensing Inspector and the Provider discussed the Health & Safety checklist. Noncompliance with the checklist was observed and was discussed with the Provider. Violations can be reviewed on the Violation Notice. Based on review of documentation and interview with the provider, the provider did not ensure that additional tuberculosis (TB) screening was obtained every two years following a chest x-ray. Evidence: The most recent TB screening submitted by the provider was dated 10/1/2021.
Plan of Correction:
A current TB screening was obtained and submitted to the Licensing Inspector on 10/26/23.

Description:
Based on review of four child records, the provider did not obtain all of the required documentation for child records. Evidence: The records for Child A, Child B, Child C, and Child D did not contain documentation of a health examination.

I have requested these from parents and will ensure I obtain physicals within 30 days of enrollment moving forward.

Description:
Based on review of documentation and interview, the provider did not meet all of the criteria on the health and safety checklist at the time of the renewal. Evidence: The provider did not have on record documentation of current rabies vaccination for a household pet.
Plan of Correction:
Rabies vaccination was obtained and submitted to the Licensing Inspector on 11/6/23.
