Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated and concluded on 5/05/2020. The director was contacted by telephone to initiate the inspection. There were 36 children present and 11 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed (3) children?s records and (3) staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on review of 3 children's records, the center did not have documentation of a physical in each child's file prior to or within 30 days of attendance. Evidence: 1. Child #3 began at the center on 1/17/2020 and the center did not have documentation of a physical for the child within 30 days of attendance..
Plan of Correction:
The parent has a physical exam scheduled on 5/07/2020 and parent will provide the documentation.
