A Subsidy Health and Safety inspection was conducted on 3/30/2023 to review supplemental health and safety requirements. There were 50 children present with 6 staff supervising.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Based on a review of five (5) staff records and interview, the vendor did not ensure that two (2) staff who worked directly with children annually completed the department's health and safety update course.
Evidence: 1. The record of staff #4 (date of employment: 10/18/21) did not contain a 2022 annual health and safety training certificate. The record of staff #5 (date of employment: 7/8/2021) did not contain a 2022 annual health and safety training certificate. 2. The vendor acknowledged that the department's 2022 health and safety update course had not been completed by the two (2) staff as required.
Plan of Correction:
The staff will obtain the training required immediately. It is a three hour course that will be worked on to obtain their certificate.
