Technical Assistance:
The notification of birth letter, issued by the hospital registrar, meets the requirement for verification of age and identity for a child.
Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on 09/09/2021 and completed on 09/15/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 3 children present with two caregivers. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 children's records and 1 caregiver record was reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on observation, the provider failed to ensure that When food is brought from home, the following shall apply: The food container shall be clearly labeled in a way that identifies the owner.
Evidence: 1. One plastic container of milk was not labeled with child 2's name. a. The provider stated that this milk was provided by the parent of child 2 for his bottle.
Plan of Correction:
The milk container will be labeled with the child's name.

Based on record review and interview, the provider failed to obtain verification of proof of the child's age and identity for all enrolled children.
Evidence: 1. The enrollment record for child 1 lacked documentation of verification of age and identity. a. The provider confirmed that she had not viewed the birth certificate or birth letter for child 1.
Plan of Correction:
The parent of child 1 has requested a birth certificate. The parent will be asked this afternoon if a birth letter is available and when the birth certificate is expected to arrive.

Evidence: 1. The Heart Saver CPR certification, for the provider, expired on June 30, 2021. a. The provider confirmed that an updated CPR class had not been completed.
