Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on May 5, 2020 and concluded on May 7, 2020. The administrator was contacted by telephone to initiate the inspection. There were 20 children present and five staff. The inspector emailed the administrator a list of items required to complete the inspection. The Inspector reviewed two children's records and two staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on record review, in one of two children's records reviewed, the licensee did not ensure that each child shall have a physical examination before the child's attendance or within one month after attendance.
Evidence: The administrator confirmed that there was no documentation of a physical examination for Child #1 (first date of attendance 4/30/19).
Plan of Correction:
The parent will bring a copy of the physical.

Description:
Based on record review, in one of two staff records reviewed, the licensee did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years.
Evidence: The Administrator confirmed that Staff #2 (date of hire 1/12/17) indicated on the Sworn Statement as to living outside of the state of Virginia within the last five years. The Administrator verified that there had been no out of state background check requested for Staff #2.
Plan of Correction:
The out of state background check will be filled out and notarized.
