Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was conducted on 3/25/2021, 4/14/2021, and 5/27/2021. A complaint was received by the department regarding allegations in the area of cleanliness, ratio, and person in charge. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation. Based on staff interview and review of documentation, the evidence gathered during the investigation supported the allegations of non-compliance with standards or law. Based on staff interviews, attendance records, and staff schedule, the facility failed to maintain the required ratio of one staff for every 10 children ages 3 years to 5 years (eligible to attend public school by September 30). Evidence: 1. On March 25, 2021 from 1:46 p.m. to 5:00 p.m., there were 16 children with one staff in the "fives" room. 2. Three children in the group were not eligible to attend public school by September 30, 2020. 3. Reportedly, the fives class has been over ratio since September as the administration was confused about the ratio for children who turn 5 after 9/30.
Plan of Correction:
Children who are not five by October 1 will be in a 1:10 ratio.

Description:
Based on review of attendance records and ages of children, the facility failed to operate within the terms of the license regarding the age range of children for whom care may be provided. Evidence: the application dated 11/2020 requested an age range from 6 months to 12 years. On 3/25/2021, there was a 3 month old child in care.
Plan of Correction:
The center director will submit a request to modify the age range of the children.
