Comments:
A mandated monitoring inspection was initiated and concluded on 6/21/2023. A focused review of items identified on the center's "Intensive Plan Of Correction" was also conducted. There were 146 children, 5 to 14 years of age, present with 14 staff supervising .The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 10 child records and 10 staff records were reviewed. Children were observed engaged in outdoor activities and games, swimming in the pool, having a dance contest, creating arts and crafts, eating lunch and boarding the buses and being escorted to vehicles as their families arrived to pick them up.The inspection started at 11:15 am and concluded at 5:00pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
* REPEAT VIOLATION * Based on a review of records and interview, the licensee did not always obtain the results of a search of the child abuse and neglect registry or equivalent registry from any state in which the individual staff has resided in the preceding five years.
Evidence: Ten staff records were reviewed. The record for staff #3, whose employment began 1/9/2023, contained a signed Sworn Statement or Affirmation form which disclosed that staff #3 resided in North Carolina within the past 5 years. The record for staff #3 did not contain a search of the state's Abuse and Neglect Registry. The program director stated that she was unable to locate this documentation during the inspection.
Plan of Correction:
The Program Director will obtain search results of any staff who has resided out of state prior to being hired in the system. Searches will be run again for those currently working in need of results by 7/14/2023.
