Comments:
An unannounced monitoring inspection was conducted on January 3, 2020 between 9:30am and 10:30am. There were 22 children in the direct care of 3 staff members. Children were observed playing group games in the large gym space and working on art projects in the classroom. The center is clean and organized. Based on record review, 2 of 5 children's records reviewed did not have documentation that a physical was conducted by a physician within 30 days of the first date of attendance.
Evidence: Child #5 and Child #6 records did not have documentation that a physical was conducted.

Description:
Based on observation, the center did not ensure that medication administered to a child had appropriate written authorization from the parent and physician.
Evidence: Medication for Child #1 was documented to have been administered by staff on May 6 2019, June 20, 2019, and July 9 2019. The authorization on file had an expiration date of October 20, 2018.
