This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on May 26, 2021 and concluded on May 26, 2021. The provider was contacted by telephone to initiate the inspection. There were 53 children present and 14 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed four children?s records and four staff records submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on review of staff records, the center did not ensure that all staff who have lived outside the state of Virginia in the past five years have a criminal history record information check and sex offender registry check from any state in which the applicant has resided in the preceding five years.
Evidence: Evidence: 1. The record for staff #4 had documentation that the staff member had lived out side of Virginia in the past five years. 2. The record for staff #4, did not have documentation of the criminal history name check and the sex offender registry check that is required for staff who have lived outside of Virginia in the last five years.
