An unannounced focused monitoring inspection was conducted on 5/15/2024 in response to a self-reported incident.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on observation, the center failed to be maintained in a safe manner.
EVIDENCE: The wooden bridge in the playground has two slats that have caved in, causing a trip hazard.
Plan of Correction:
The slats are scheduled to be replaced this weekend, and in the meantime, teachers stand at the broken spot to keep children away from it.

Description:
Based on observation, the center failed to have sufficient resilient surfacing and fall zones on playground equipment.
EVIDENCE: The resilient surfacing (playground wood chips) measured 4 ? inches deep under the monkey bars, which were 7 feet high. Use zones around the monkey bars were less than six feet ? in some areas the use zone of the zip line was directly under the monkey bars. Use zones were not at least 6 feet around the outside perimeter of the play structure.
Plan of Correction:
Children will not be allowed to use the equipment until surfacing and use zones are corrected. The playground surfacing will be measured daily and fluffed up with a heavy fork. Additional surfacing will be purchased as needed. The zip line and 360-degree swing ?wrecking ball? will be removed before children can use the equipment. The equipment will be moved and/or boundaries expanded for a use zone of at least 6 feet on all sides of the equipment. An allowable variance will be requested because some use zones will be a bit shy of 6 feet.
