Technical Assistance:
Consultaiton provided on background checks and crib compliance.
Comments:
An unannounced renewal inspection was conducted with the voluntarily registered provider, who was present with 2 children initially with the third child arriving during the inspection. The ages of the children were 17 months, 18 months and 4 years of age. Three children's files were reviewed today. The caregiver reported no child had medication on site and there have been no injuries/accidents. Four household member's files were also reviewed today.
Please let me know the date you send off for all of your background checks, which should be no more than 10 days from the date of your inspection. Please send me the documentation of the completed background checks.
You cannot use cribs that do not meet the current Consumer Product Safety Commissions guidelines. Your current cribs cannot be used and should be removed from the daycare area.
The areas of non compliance is outlined on the violation notice.
Based on interview and a review of recordkeeping, it was determined that 4 household members have not obtained repeat central registry background checks every five years. Evidence: Person A, B, C and D's central registry background checks are dated 8/5/16 and have not been renewed within 5 years.
Plan of Correction:
We will send off for the repeat central registry background checks for Person A,B,C and D.

Description:
Based on interview and a review of record keeping, it was determined that 4 household members did not have sworn disclosure statements on file upon certification. Evidence: There were no sworn disclosure statements on file for Person A, B, C and D. The caregiver has been voluntarily registered at least since 2018.
Plan of Correction:
We will have Person's A,B,C and D complete sworn disclosure statements.

Description:
Based on observation, interview and a review of record keeping, it was determined that the provide does not meet the criteria on the health and safety checklist at the time of the renewal inspection. Evidence: 1. Three children's files were reviewed and there was no physical on file for child A within 30 days of enrollment. Child A enrolled on 8/16/21. 2. The caregiver had two cribs in the daycare area. There was no documentation provided to show that either crib meets the current Consumer Product Safety Commission guidelines for full size cribs. There was no documentation showing that either crib was manufactured in 2010 or later. 3. The caregiver is not documenting monthly practiced emergency evacuation drills.
Plan of Correction:
We will not use the cribs until we are sure they meet the current federal requirement. We will start documenting monthly escape drills. We will obtain the physical for child A.
