Comments:
The licensing inspector conducted an unannounced inspection in response to a complaint that was received by the department on May 12, 2021 regarding ratios, supervision, and the center's programs. The investigation was initiated on May 12, 2021 and completed on September 22, 2021. Interviews were completed, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports some of the allegations, therefore the complaint is determined to be valid. See the violation notice for more details.
