Comments:
An unannounced renewal inspection was conducted today. There were 27 children present with 3 staff supervising. The physical plant, accident logs, attendance logs, fire inspection report, menu's, first aid kit, medications and authorizations, children's records, staff records and emergency preparedness procedures were reviewed. The children were observed working on crafts, playing board games and talking in small groups. Areas of non-compliance were cited on the Violation Notice. Based on a review of children's records, the center failed to obtain all required documentation for each child enrolled.
Evidence: Child #5's record does not contain documentation of a place of employment for each parent who has custody.
