Comments:
An unannounced Monitoring Inspection was conducted today from 10:45am to 11:30am. There were 17 preschoolers directly supervised by 3 staff. The physical plant, 3 staff records, 6 children?s records, 1 child's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play and performing in a holiday concert. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid. Staff-to-child ratios were in compliance. The center was clean and organized. Based on review, the facility did not maintain children's records with the required information. Evidence: 1) Reviewed 6 children's records and found that Child 3, 5 and 6 did not have complete work information for one or more parents. 2) Reviewed 6 children's records and found that Child 3 only had 1 emergency contact and Child 5 did not have an address for one of their emergency contacts. 3) Reviewed 6 children's records and found that Child 2, 3, 4, 5, 6 did not have documentation of their first date of attendance.

Will contact parents and get them and will pull the rest of the files and double check them.
