Comments:
A monitoring inspection was conducted today from 10:25 am to 11:10 am. There were 9 children (14 points) in care of 2 caregivers. The children were observed playing with toys and playing outside. Records for 2 caregivers, 2 children and 2 household members, and attendance was reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only. Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website. Based on staff interview, the family day home did not obtain a repeat background check for an adult household member, every five years. Evidence: 1. Household Member # 1's most recent central registry search (CRS) was dated 10/3/18. CRS have to be repeated every five years. 2. The provider stated that a repeat CRS for Household Member # 1 was not submitted.
