Comments:
A monitoring inspection was conducted on June 24, 2024. The inspector was on site from approximately 9:00 am until 11:40 am. The director was present during the inspection. There were 28 children present, ranging in ages from three years to 12 years, with seven staff Supervising. Children were observed in a variety of activities including art (creating pirate ships, mermaid tails and crabs), building with magnetic blocks, circle time with music and bells, puzzles, and creative play with a grocery store center. Interactions between staff and children were positive. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and five staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of five staff records and interview, the center did not obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment.
Evidence: 1. The record of staff #2, employed 11/08/2023, contained documentation of a negative tuberculosis screening dated 11/10/2023, exceeding the first day of employment. 2. The director acknowledged the TB results for staff #2 were documented after the first day of employment.

Per the director: Staff will obtain TB screening results prior to employment.

Description:
Based on review of five staff records, the center did not ensure one staff obtained and submitted the results of a follow-up tuberculosis screening at least every two years from the date of the initial screening.
Evidence: The record of staff #5, employed 12/28/2011, contained documentation of a tuberculosis screening dated 06/01/2022, exceeding two years. The administrator stated a follow-up tuberculosis had not been obtained.

Per the director: Staff #5 will obtain a current TB screening upon returning for vacation.

Description:
Based on review of medication logs and medication authorizations, the center did not obtain written authorization from a parent or guardian before administering prescription medications.
Evidence: 1. The medication log documented that child #9 was administered a prescription medication from 06/05/2024 through 06/21/2024. 2. The medication authorization on file for child #9 contained only the first page with the physician?s authorization. The second page was missing and there was no authorization from the parent or guarding to administer. 3. The director acknowledged the medication authorization form was incomplete and did not contain the parent authorization.
Plan of Correction:
Per the director: I will have the parent complete the second page of the authorization for the file.
